All data were de-identified to be compliant with Health Insurance Portability and Accountability Act and Office of Health Promotion Research regulations for purposes of secondary research use. The structure of the COTA database has been reviewed by the Western Institutional Review Board and has been deemed exempt from patient informed consent.
